First I created a new layer and put it beneath the sig, then filled it with black so I could see the sig on a dark background. Then I added a mask to the sig layer, and used the rectangle marquee selection tool to select the bottom of the nurse where I wanted it to fade. I used a white to black gradient with the gradient fill tool to fill in the the selection on the mask top to bottom.

oh yeah I forget to mention that I then turned off the bottom layer that is filled with black before saving
